New Patients

We are pleased to have new families join our practice.  Please read below for helpful information about your child's first visit with us.

First Time Parents

Couples expecting their first child may wish to schedule a complimentary "Prenatal Visit" to meet one of our physicians prior to the baby's arrival.  This visit will allow you to see our office, get acquainted with your new pediatrician, and ask questions about our approach to child care.  Please call to make an appointment for this visit.  If you are unable to come for a "prenatal visit"  you may still choose one of our physicians by naming him or her as your pediatrician if the baby is born at Northside Hospital.  If you deliver at another hospital, call for an appointment with one of our doctors when your infant leaves the hospital.

Experienced Parents

Parents of children who have recently moved to our area or who are changing physicians for other reasons should call to schedule a "New Patient - Well Child Exam".  It will be very helpful if you can bring immunization and growth records for us to review during your first visit.  Also, if possible, please complete the registration forms below.

Parents of Children with Special Medical Needs

If you are new to our practice and your child has any serious or ongoing medical problem, please inform us of this when making your first appointment.  We will need to schedule sufficient time to review your child's medical history and address active medical problems.  You should plan to bring pertinent medical records and all of your child's prescriptions with you.  Also, if possible, please complete the registration forms below.

Registration Forms

All new families will need to complete some paperwork before the first visit with the physician.  Forms which we will ask you to complete are available below.  (You will need Adobe Reader to print these documents.  Click the icon below to download the reader.)  

We would appreciate receiving a copy of your registration information by mail or fax at least 2 days prior to your appointment.  You may fill in the blanks on the screen and then print the document to fax to us at 404-256-1759 or mail it to us.  

If you are unable to send this information before your appointment,  we ask that you arrive 30 minutes early to allow extra time for registration.  Thank you.
